Application forms and guidelines
To apply for an SMK Campaigner Award you need to complete and submit an application form (available for download below) by 1pm on Monday 17th June 2013.
You can submit the form by emailing it to us at awards@smk.org.uk, or by posting it to 'Awards', Sheila McKechnie Foundation, Resource for London, 356 Holloway Road, London N7 6PA.
Please make sure you have read through the application guidelines, which can be downloaded below, as well as checked the application criteria and FAQs before submitting your application. Please also see this table for a quick overview of the award categories.

Please note that the Shout Out! Award (for campaigners aged 14-18) has a different application form and guidelines to the standard one. Please ensure that you complete and submit the correct application form.

Shout Out! Award application form
The Shout Out! Award is for 14 - 18 year olds based in London. If you are 17 or under you will need to have your application form signed by your parent or guardian. You can then scan and email or post the form to us. You also have the option of submitting a video application; for more information, please see the guidelines below.

We will acknowledge receipt of all applications by email. If you do not receive a response from us within ten working days please email or call us on 020 7679 4040 to check that your application has been received.
